Confirmation of Enrollment and Roommate Selection Form This form confirms your intent to enroll at Houghton and must be accompanied by a $300 enrollment deposit.  We also use this to assist with roommate placement. Download form
Final Transcript Request Form Give this form to your guidance/registrar's office in the summer to request a final transcript. Download form
Health Forms
Confidential Medical Report Complete medical information and immunization records are required for all Houghton students. Download form
Intercollegiate Sports Health Form This form is only required for those planning to participate in intercollegiate sports at Houghton. Download form
Off-Campus Program Health Form This form is only required for those participating in the Highlander Wilderness Adventure Program or the First-Year Honors Program. Download form

Having trouble printing the PDF forms?

If you experience difficulties in downloading the above forms try one of the following:

  1. Right-click on the link, select "Save Target As..." and save the file to your desktop. You should now be able to open and print the file.
  2. Call or e-mail the Admission Office and we will mail or fax you a copy of the form you require. (Be sure to specify what form you need and your mailing address or fax number.)
