Application Checklist

• Meet with an Enrollment Advisor to determine eligibility

• Request Official Transcripts from ALL colleges and universities attended

• Complete two reference forms (one personal and one professional)

• Complete Admission Essay

• Complete FAFSA form for Financial Aid

• Immunization Form

• If military experience, submit military/ACE transcript and DD-214
